Your Appointment

We look forward to being of assistance to you on your first visit with Hollingshead Eye Center in Boise.

Your first visit with us could last an hour and a half to two hours, so please plan accordingly.

We do take a photo of every patient to identify them with their medical record.

Please bring the following with you to your appointment:

• Completed new patient forms
• A list of all prescribed medications with dosages and quantity
• Insurance card(s)
• A valid photo ID (preferably a Driver’s License)
• Payment in the form of check, cash, or credit card

We know that sitting in a reception area filling out forms can be stressful. It is so much easier to recall all of the details you need to answer those questions when you can fill out those forms at home and bring them with you.

We do everything we can to make your visit to our office as hassle-free as possible. Downloading and printing out these forms on your home computer and filling them out ahead of time is one way to do that.
